Sounds easy enough.
You need to break this down into 2 parts. The first is the form the potential client must fill out.
This can be accomplished by using phpFormGenerator. http://phpformgen.sourceforge.net/
The second part is not exactly as wished with coded mailboxes but a Content Management System (CRELoaded) does exactly as wanted but with account names and logins.
When the professional is assigned the client he logs in with his admin account and makes a client membership in the website's admin. Using this process he sends the client the log in information. A full record of all correspondence is kept on the website along with dates and times.
With this system all admins could see all correspondence.
A more discrete system can be made with customer management software. Any of the 3 below would work.
