Two sites I always cite as examples of what not to do:

1. www.halliburton.com - Includes a particularly awful implementation of cascading menus, demonstrating why they're not a necessarily good thing. There are abundant other problems with the site if you take the time.

2. www.fanniemae.com - Inconsistent design, poor navigation structure and behavior, illegibly small text, disregard for established web conventions, the list goes on and on.

What's also good about these is that it demonstrates that even giant corporations can have ridiculously bad web site implementations. They both even have clearly expensive designs, but suffer for lack of usability.
